LMHC, LMFT, MSW) to provide assessment, diagnosis and formulates documentation and implementation of treatment plans addressing the mental and behavioral health of patients and their families, case management, education, and advocacy to patients and their families. Will assist in identifying and resolving difficulties from a bio-psycho-social perspective and communicates and collaborates with other healthcare providers and social service providers when appropriate.
